A letter of intent is a document that gives future caregivers a written wishlist for how a certain end of life measure is carried out. This is not a legally binding document, more of a suggestion. This letter will communicate with caregivers, guardians, trustees, and advocates of dependents. This letter would be used to list anything that you wish to be done before or after your death.
Your letter of intent should be laid out into several sections. This includes:
- Biographical and personal information such as name, date of birth, and religious preference
- Medical
- Legal
- Educational
- Financial
- Contact information
- Vision for your loved ones future
Points that should also be included:
- Description of your loved ones diagnosis

- What they like to do for fun
- Their strengths and weaknesses
- Their education and work history
- Caregiver’s choice of successor caregiver/guardian

It’s easy to find templates online to help you get started. It may also help to watch some videos to make it easier. Make sure to involve others as well. Have a family meeting to discuss what best suits everyone. Make sure they know what you want and specifically how to carry it out. The more people you involve the better. This can ensure your plan gets carried out the way you want it to.
